Q. I am 71y, diabetic for 12 yrs, no htn,no signs of IHD so far, strong FH of Dib and CHD, not obese, BMI 23, daily walker for 2-3 km, well managed with Metformin and Semidionil for last 3-4 yrs with FBS 92 t0 120 but PPBS of 200 to 220, HBA1c 7.7 6 mths ago. Recenly had mild chest uneasyness and had to have CABG due to +ve TMT and Left main lesion and LAD block but no infarct about 2 mths ago and doing fine now. Post operatively my Physician changed my diabetic regimen switching to Amaryl 2mg B.D saying for Cardiac patients this is better than Semidiaonyl. But with Amaryl and Metformin, my diabetic control is worse with FBS 125 to 156 and PPBS 220 to 280. Doctor says he has to titrate the dose. My question is "Is semidionil not safe for cardiacs. Ia m afraid of taking max dose of amaryl while lower doses of Semidionil was working better before?". Thanks
A. The subject is controversial., however if properly titrated insulin in combination with metformin and if required Amaryl will be the best in your case.Keep your A1c below 6.5.Post prandial highs are not good for heart.

Q. Dr., how can I manage Diabetes and chrohn's disease together?
A. We do have a couple of them on follow up.You should be on regular follow up with a gastroenterologist and also with a diabetes team.Both diseases are not curable but controllable.The experts will decide on drugs which will not have interactions and side effects.Once in 4 months it is better to undergo lab and physical evaluations. www.jothydev.net, jothydev@vsnl.com

Q. how to control cholesterol
A. Diet, exercise and if advised by doctor medications.High cholesterol is a risk factor for heart attacks, strokes, kidney disease and diabetes.If you can control cholesterol you can avoid most of the dealy diseases.Worth taking medications under your doctors advise.

Q. Now a day PCOD is a common problem faced by the young girls. Is this is another risk factor for diabetes?.
A. 100%.We have plenty of young girls being treated at our center, with PCOD and diabetes due to insulin resistance.Getting prepared for pregnancy in an intelligent fashion is the most critical period.

Q. I walk 1 hour daily and diabetics is under control for 10 years.Should I worry about kidney disease od diabetic?
A. Daily walking is good for diabetes.However kidney disease is related to average sugar, Blood pressure, lipids , duration of diabetes etc.,Meet your diabetes team and be regular in doing all tests for the important organs,at least once in 6 months.

A. you should do PFT/PEFR and there are good inhalation medications.Though not curable your allergic asthma is completely controllable.

A. I have several young patients with diabetes and obesity who cannot get married due to their illnesses.I sincerely wish young educated youth either healthy or without the same illness to get registered in this site and start knowing each other and perhaps later get married...

A. Type 1 is spontaneous and requires life long insulin.Type 2 is the commonest diabetes and occur in people who are overweight with added risk factors like high lipids, lack of exercise, genetics etc.,

A. Diet depends on body weight, previous sugar, cholesterol status,medications etc., Instead of suggesting a general diet, get in touch with the dietician of your treatment team.

Q. I am newly diagnised to be having diabetes.I am 29 yrs old ,65 kgs,5 feet tall.What is the best way for me to tackle this disease?
A. Ahsa: Do tests to differentiate type 1 from type 2.(Ce peptide, Anti GAD etc.,)You should be in constant touch with a trained diabetes team so as not to develop any complications in future.Just by taking some medications daily will not be a solution.
